国際口腔保健政策と日本の歩み：
高齢者ケアの未来を見据えて
日本は世界に先駆けて超高齢社会を迎え、歯科医療における日本の役割は一層重要となっています。本セッションでは、日本の歯科界の国際的貢献、特にFDIやOHAP、UHCなどの枠組みを紹介するとともに、国内のフレイル対策や地域包括ケア、海外の高齢者歯科の取り組みを共有し、これからの高齢社会における歯科医療の方向性を示します。

2015年に、健康寿命延伸のための歯科医療・口腔保健 世界歯科会議（日本歯科医師会主催WHO共催）が開催され、閉会時に「東京宣言」が発表された。この宣言は、高齢社会における歯科医療・口腔保健の課題を共有し、口腔の健康実現に向けた枠組みを構築するため骨子を示すものであった。
その後、GCのサポートを受けて、議論の場はFDIに移り、2016年に「高齢社会における口腔保健（Oral Health for an Ageing Population：OHAP）」プロジェクトを推進するタスクチームが発足した。
2026年は、このプロジェクトが10周年を迎えた年であり、本講演ではその成果と今後の展望について紹介する。

8020運動により日本の高齢者の現在歯数は大きく改善し、次の課題として加齢に伴う口腔機能低下への対策が注目されている。これを背景に、日本ではオーラルフレイル概念が提唱され、軽微な口腔機能低下が全身のフレイルへ連鎖することが示された。さらに口腔機能低下症の公的医療保険導入により、日本の歯科医療は歯科疾患（う蝕・歯周疾患）予防・治療に加え機能管理も重要な視点として加わった。本講演では、日本のこれまでの経緯をもとに高齢社会における口腔機能管理の意義を考察する。

Global Trends in Oral Health and Ageing Populations
Globally, the population is ageing with dramatic demographic shifts evident in many countries around the world. As our populations change, healthcare delivery needs evolve with many funding models becoming increasingly outdated and unsustainable. Alongside these changes to population structures, the oral health profile of older adults has also changed with many countries seeing a change from an edentate to a partially dentate older population. Whilst this is very positive, it does present the dental team with the challenges of managing ageing dentitions in patients who may also be suffering from a range of chronic systemic diseases. As we embrace the concept of “healthy ageing” this presentation will present pragmatic approaches to care, and draw on experiences from the United Kingdom and Ireland in delivering dental care for dependent older adults. The prevention and management of caries in older adults will be discussed in addition to simple tooth replacement strategies.
