The management of malocclusions has evolved significantly with the integration of skeletal anchorage systems and clear aligner therapy. Contemporary orthodontic practice combines traditional biomechanical principles with innovations such as miniscrews (temporary anchorage devices) to enhance control, efficiency, and predictability of tooth movement. This lecture reviews the principles of anchorage management, indications for miniscrew-assisted mechanics, and the biomechanics of aligner-based treatment. Emphasis will be placed on case selection, treatment planning strategies, and clinical protocols that optimize outcomes in different malocclusions. Clinical examples will illustrate how anchorage reinforcement and aligner therapy can be synergistically combined to expand therapeutic possibilities while maintaining minimally invasive and esthetic approaches.
