What is the formula for successful periodontal therapy?
Why do outcomes differ even when the same treatment is performed? How can we overcome cases of periodontal disease that do not respond to treatment? This session explores these questions and the factors that contribute to successful periodontal care. Part 1 focuses on the science behind successful periodontal therapy, including biofilm control, comprehensive treatment planning, and evidence-based non-surgical treatment. Part 2 highlights practical strategies for motivating patients, delivering effective oral hygiene instructions (OHI) based on gingival assessment, and making sound clinical decisions in everyday practice.

The Goal of Periodontal Therapy is the Control of Biofilm Pathogenicity

The onset of periodontal disease is driven by the increased pathogenicity of biofilm.
Biological shifts toward higher pathogenicity in biofilms.
Therapy aimed at reducing pathogenic bacteria and increasing beneficial bacteria

The primary cause of periodontal disease lies in the increased pathogenicity of biofilm. The oral cavity is inhabited by a mix of pathogenic (bad), beneficial (good), and opportunistic bacteria. Periodontal disease develops when bad bacteria proliferate and good bacteria decrease. The ultimate goal of periodontal therapy is to eliminate only the bad bacteria without reducing the harmless ones. The elimination of the etiology involves improving the subgingival flora and lowering its pathogenicity. In this lecture, I would like to discuss periodontal treatment approaches aimed at etiological elimination from a biological perspective.

In many OHI (Oral Hygiene Instruction) sessions, the focus is placed on how to remove disclosed plaque—the “red” areas—and on the tools and techniques used to do so. However, the true goal is not to achieve 100% plaque removal, but to treat periodontal disease. What truly matters is the ability to identify why plaque remains, uncover the underlying problems, and find effective solutions. This is what we call “prescriptive OHI.” By sharing treatment goals with patients, we can encourage meaningful behavioral change. This is the formula for successful periodontal therapy. Let’s move on to the next stage together.
