In periodontal therapy, a patient-centered team approach involving both dentists and dental hygienists is an important factor in achieving successful treatment outcomes.
In this session, four clinics will share their approaches to periodontal care through long-term clinical cases, highlighting the roles of dentists and dental hygienists, methods of information sharing, and ways to build trust with patients. The speakers will also share practical insights into common challenges faced by clinics providing periodontal treatment and how they address them.

The prognosis of patients with severe periodontitis depends not only on the surgical procedure itself but also on the quality of teamwork in everyday clinical practice. Based on an 8-year follow-up case, this lecture will discuss inflammation control through OHI and non-surgical periodontal therapy as the foundation of treatment, followed by alveolar bone recontouring through resective therapy and its contribution to postoperative stability. Prosthodontic treatment planning based on a comprehensive evaluation of the dental arch and missing teeth will also be presented. In addition, the lecture will highlight the dental hygienist's role in OHI, SPT, and patient management, as well as effective collaboration with the dentist, to demonstrate the key factors for achieving long-term periodontal stability.

In patients with severe periodontitis, complete elimination of tooth mobility is often difficult to achieve. Therefore, successful long-term management depends on controlling tooth mobility throughout initial periodontal therapy, oral rehabilitation, and SPT. Following inflammation control through initial periodontal therapy and periodontal surgery, mobile teeth are evaluated and managed using temporary and permanent splinting. During SPT, patient-centered approaches that take each patient's individual background into account play a key role in preserving teeth over the long term. Using a clinical case of a smoker, this lecture will summarize the key intervention points for both dentists and dental hygienists.

It takes time and experience for a dental hygienist to become capable of managing patients with severe periodontitis. This requires developing the ability to assess the difficulty of periodontal cases, evaluate bone defects and furcation involvement on a tooth-by-tooth basis, and acquire the appropriate clinical skills to manage them. As periodontitis becomes more advanced, knowledge of prosthodontic treatment and the management of partially edentulous dentitions also becomes increasingly important. Patients cannot wait for dental hygienists to develop these skills, so the question is how the dental practice can establish a step-by-step process that supports their professional growth. Furthermore, if patients’ values and level of interest have a significant influence on the long-term course of periodontal disease, perhaps clinicians are also called upon to grow not only professionally, but as individuals.

Long-term follow-up cases represent invaluable clinical evidence. By reviewing treatment outcomes during the maintenance phase, we can identify the clinical approaches and treatment trends adopted at different points in time and gain new insights from their long-term results. These cases also help us understand when and how treatment should be provided based on each patient's personality, lifestyle, and overall health status. In this lecture, we will present the long-term clinical histories of patients under our care and discuss the rationale and timing of the treatment interventions carried out throughout their course of care.
