  • Biological processes underlying the progression and healing of periodontitis, and the fundamental principles of periodontal regeneration
  • Success factors and limitations of periodontal regeneration based on current clinical evidence
  • The role of biomaterials in periodontal regeneration and their optimal clinical application based on material characteristics
  • Future directions in periodontal regenerative therapy
The success of periodontal therapy depends on a thorough understanding of biological principles and precise clinical decision-making tailored to each individual case. This lecture will review the biological basis of periodontal regeneration and the wound-healing process, followed by an overview of current recommendations and supporting evidence from major professional organizations. Success factors and limitations of periodontal regenerative therapy will be discussed from an evidence-based perspective. Particular emphasis will be placed on the clinical application of biomaterials, including enamel matrix derivative (EMD) and fibroblast growth factor-2 (FGF-2), in patients with severe alveolar bone defects. Their indications, use in combination with bone graft materials, and the latest evidence regarding drug delivery effects will be critically examined. By considering biomaterials as tools for biological optimization, this lecture will explore the importance of modulating host responses from the perspective of precision periodontology and discuss how current evidence can be translated into optimal clinical practice.

Advances in periodontal regenerative therapy have made outcomes more predictable. A significant recent development is the flap design that avoids_interdental papilla incisions, enabling access to bone defects_from the buccal side_through a tunnel approach. To expand its indications, we introduced the Double Sided Entire Papilla Preservation Technique. Additionally, we developed the Supra-alveolar Periodontal Tissue Reconstruction Technique to improve multiple vertical hard and soft tissue defects, previously considered impossible to address. This presentation focuses on hard and soft tissue management in the esthetic zone, showcasing these techniques for both natural teeth and peri-implant tissue regeneration.
