The widespread adoption of immediate implant placement and ridge preservation has reduced the need for bone augmentation procedures. Nevertheless, bone augmentation remains indispensable in selected clinical situations where adequate bone volume is essential for successful implant treatment.
As clinicians strive to achieve more predictable and advanced outcomes, effective risk management has become increasingly important for ensuring long-term treatment success. In this lecture, the long-term outcomes of bone augmentation cases will be reviewed to identify factors that contribute to treatment stability. Surgical techniques will be re-evaluated from the perspectives of graft material selection, flap management, and the choice of surgical approach according to the anatomical characteristics and extent of the bone defect.

GBR Treatment Strategies for Long-Term Success: Defect Assessment, Surgical Technique Selection, and Long-Term Outcomes

To achieve long-term implant success, it is essential to clearly define the required bone volume and morphology before augmentation, and to establish appropriate treatment goals based on a three-dimensional assessment of the bone defect. This presentation will outline the surgical selection process for GBR procedures in mild, moderate, and severe bone defects, with careful consideration of the balance between predictability and invasiveness. Furthermore, we will discuss, through clinical cases, that the achievement of long-term prognosis requires not only skeletal stability, but also harmonization with the surrounding soft tissues and the establishment of adequate cleansability.

Rethinking Sinus Lift Surgery: Clinical Effectiveness, Complications, and Long-Term Stability

Sinus lift surgery is an internal bone augmentation procedure that enables placement of implants with a success rate exceeding 95%, even in cases with minimal residual bone height where vertical bone augmentation using guided bone regeneration (GBR) would otherwise be required. Reports indicate that sinus lift surgery is necessary in approximately 40–50% of implant cases in the posterior maxilla, making it one of the most frequently performed bone augmentation procedures.
However, the degree of surgical difficulty varies considerably depending on the clinical case. Intraoperative complications such as Schneiderian membrane perforation and bleeding may occur, while postoperative complications include infection, maxillary sinusitis, and rhinosinusitis. In the long term, peri-implantitis and perigraftitis may also compromise treatment outcomes. Through a series of clinical cases, this lecture will discuss practical strategies for achieving predictable sinus lift surgery while minimizing complications and maintaining long-term stability.
