Periodontal Surgery Aiming for Tissue and Esthetic Reconstruction

Successful management of complex periodontal surgery requires a variety of treatment approaches. In this session, we welcome leading clinicians in the field to review the evolution of periodontal surgery from past to present, while sharing and discussing the concepts and techniques that lead to better treatment outcomes.

Flap Design in Minimally Invasive Periodontal Surgery: An Evidence-Based Approach
- The importance of flap design beyond surgical access: Its impact on wound healing, blood clot stability, and soft tissue preservation, supported by current evidence
- Key principles of flap design common to minimally invasive periodontal surgery and how they differ from conventional techniques
- Clinical evidence on the effectiveness and limitations of minimally invasive flap designs, and their appropriate application according to the clinical situation
Various flap designs have been proposed based on these biological principles, each with distinct advantages and limitations. Understanding the rationale behind flap design is therefore essential for achieving predictable clinical outcomes.
This lecture will review the fundamental concepts shared by minimally invasive periodontal surgical procedures and highlight their differences from conventional flap approaches. Furthermore, evidence from both clinical and basic studies will be presented to evaluate the effectiveness and limitations of minimally invasive flap designs. Finally, practical considerations for selecting appropriate flap designs according to defect characteristics and clinical situations will be discussed.

- Although periodontal regenerative therapy is a well-established and evidence-based treatment modality, its predictability varies according to defect characteristics, and certain clinical situations remain challenging.
- Tunnel technique–based periodontal regenerative therapy has emerged as a valuable surgical approach and can provide highly predictable outcomes when appropriate case selection is applied.
- A thorough understanding of the indications, advantages, and limitations of the tunnel technique is essential for achieving optimal clinical outcomes.

This lecture will discuss strategies for periodontal regenerative therapy in the posterior region with a focus on soft tissue management. First, the CTG Wall Technique for the treatment of furcation defects will be introduced. This technique uses a connective tissue graft to create a soft tissue wall, aiming to improve the predictability of regenerative therapy by maintaining the regenerative space and stabilizing the blood clot.
Furthermore, the Last Molar Entire Pad Preservation Technique (L-EPPT), developed by the author, will be introduced for sites with significant anatomical constraints, particularly the terminal molar region. This technique avoids incisions in the retromolar pad area and preserves the blood supply, thereby promoting wound healing and achieving primary closure.
Through clinical cases, this presentation will demonstrate the indications and key points of incision design for each technique and discuss the role of soft tissue management in improving the predictability of periodontal regenerative therapy in the posterior region.
