Why does hard calculus form so quickly on the CEJ, even though textbooks depict it as smooth?
For example, on the distal surface of the last molar, there may be a deep pocket without pus discharge, and calculus may be present but impossible to remove. Furthermore, when debridement is performed on the mesial surfaces of maxillary premolars or mandibular canines but the pocket remains, how far does a Grade III enamel projection extend into the furcation?
Do you encounter situations like these in your daily clinical practice?
In this session, I would like to explore these possible causes from the perspective of root anatomy using 3D images.

Root surface detection is an extension of the periodontal examination, and successful periodontal therapy depends on a systematic approach based on clinical findings rather than blind instrumentation. This lecture will demonstrate how to integrate the patient's chief complaint, radiographic findings, intraoral photographs, and knowledge of root anatomy to develop a mental image of the subgingival root surface before instrumentation begins. The lecture will also discuss the use of dedicated exploratory instruments for accurate calculus detection to avoid unnecessary SRP, appropriate decision-making during re-evaluation, and practical techniques for improving root surface detection skills that can be readily applied in daily clinical practice.

Have you ever felt that certain areas remain difficult to debride during SRP, such as deep periodontal pockets or sites with complex root anatomy? This lecture will identify the factors that make instrumentation challenging and explain practical approaches that can be applied in daily clinical practice. Particular attention will be given to furcation involvement, an area in which many clinicians lack confidence, by reviewing its anatomical characteristics and organizing the clinical reasoning and technical steps required during SRP. In addition, considering the limitations of mechanical plaque control, the lecture will focus on improving assessment and intervention skills that lead to better treatment outcomes, greater clinical confidence, and more precise maintenance care.
