The long-term prognosis of endodontically treated teeth depends not only on the quality of root canal treatment but also on the quality of the subsequent restoration, with root fracture representing one of the major causes of failure. This lecture outlines treatment strategies that emphasize the effectiveness of one-day treatment and the reduction of flare-ups. In addition, the biomechanical advantages of fiber posts over metal cores and the role of increased bonding surface area in reducing the risk of root fracture will be discussed based on the literature. Clinical cases of minimally invasive restoration for teeth considered difficult to preserve will also be presented to explore the potential for avoiding extraction.

A systematic review reported that Akiyama was the first to demonstrate successful outcomes in papilla reconstruction. This treatment concept was established more than 20 years ago and has continued to evolve. While contemporary dentistry is largely disease-oriented, tooth loss cannot be fully explained by disease alone. The Micro Root Recovery Akiyama System is a novel treatment concept for attachment loss focusing on root structural defects. It is based on the Three-Step Akiyama Method and microscope-assisted dentistry as its fundamental principle. In this presentation, a new treatment strategy for attachment loss focusing on root structural defects will be introduced.
