Suction Dentures in the Era of Severely Resorbed Ridges

With advances in periodontal therapy and increasing life expectancy, the number of edentulous patients presenting with severely resorbed mandibular ridges has become increasingly common in clinical practice. In these patients, achieving adequate denture retention and stability remains a significant challenge.
This lecture will review the seven key characteristics of severely resorbed mandibular ridges and their corresponding clinical management, with a focus on mandibular complete denture fabrication based on the concept of suction. Particular emphasis will be placed on changes in the morphology of the floor of the mouth during mouth opening—whether it elevates, descends, or maintains its shape—which represents a key factor in achieving successful denture suction.

  • Maintenance is an essential component of full denture therapy for preserving and restoring oral function.
  • Incorporating the assessment of oral hypofunction enables objective evaluation of function, facilitating functional management and patient guidance.
  • As healthcare systems place greater emphasis on prevention and long-term management, integrating functional management into complete denture care is becoming increasingly important.
In today's super-aged society, complete denture therapy requires not only the fabrication of the "hardware" (denture design), but also the continuous management of the "software" (oral function). This lecture will first outline a systematic approach to mandibular suction dentures, from clinical examination to definitive impression techniques, and then introduce a practical approach that incorporates the assessment of oral hypofunction into maintenance care. The lecture will also discuss the application of the 2026 revision of the Japanese dental fee schedule and collaboration with dental hygienists, highlighting a practical approach to complete denture therapy that extends beyond denture fabrication.
